Reliant Care Management Company is seeking an LPN to provide compassionate nursing care for residents in our facility in Pennsylvania. The role requires performing comprehensive assessments, admissions, discharges, wound care, and emergency responses, while leading CNAs and CMTs to ensure protocol adherence.

The ideal candidate holds a valid LPN license and demonstrates strong documentation and teamwork skills to support resident well-being through meticulous care and communication.
