Reliant Care Management Company seeks an LPN in Andover, MA to support resident care with empathy and professionalism. You will perform admissions, ongoing assessments, wound care, and care coordination under facility protocols.

Lead CNAs and CMTs while ensuring compliance and quality care, and respond promptly to emergencies. Licensed practical nurse licensure is required to join our team.

Qualifikationen

• Must demonstrate genuine empathy and patience in resident care.
• Strong work ethic, integrity and competence required for the role.
• LPN licensure in good standing is mandatory.

• Conduct assessments, follow protocol, report changes and document findings.
• Perform admission duties: assessments, MAR/TAR/POS review, orders, meds.
• Manage discharges: documentation, notify physician/guardian, arrange transport.
• Provide wound care following protocols and dressings as needed.
• Lead CNAs and CMTs ensuring facility policies are followed.
• Respond effectively to emergency situations.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
