365 is seeking LPNs with availability for days, evenings, or overnights in Northeast Philadelphia and Lower Bucks County.

Responsibilities include:

1. Data collection on patients' physical, psycho-social, emotional, family, support system, and environmental status for home care suitability.
2. Following the written plan of care developed by registered nurses and the healthcare team.
3. Identifying patient care needs and reporting them to the Clinical Supervisor.
4. Coordinating care with other healthcare team members and vendors.
5. Complying with documentation, communication, and infection control policies.
6. Performing skilled nursing procedures as ordered, providing patient and caregiver education, and documenting care accurately.
7. Using equipment safely and organizing assignments effectively.

Requirements:

• LPN or RN license (required)

Job Details:

• Full-time, Part-time, PRN, Per diem, Temporary
• Salary: $30.00 - $38.00 per hour
