Job summary

Aviata at Orange Park is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in our health and rehabilitation center in Orange Park, FL. This full-time role offers day and night shifts, collaborative team environment, and opportunities to grow your clinical skills.

You’ll work under a nurse supervisor, participate in care plans, supervise CNAs, and help implement treatment plans while maintaining high standards of patient safety.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Compassionate, dedicated to high-quality patient care.

Responsibilities

• Assist in implementing individualized treatment plans for each resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services to residents and patients.
• Conduct and document evaluations of medical status at admission and during treatment.
• Other duties as assigned.

Major Responsibilities

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
