Job summary

Interim HealthCare in Ohio is seeking an LPN for weekend day shifts to provide one-on-one in-home nursing care. This role requires at least one year of nursing experience and an active LPN license in Ohio. The position offers Saturday and Sunday day shifts (10–12 hours) with an option for an additional weekday shift.

You will perform assessments, administer medications, document care, and educate families while collaborating with the clinical team to ensure high-quality patient-centered care.

As a Home Health Licensed Practical Nurse, Here's What You'll Do:

• Provide skilled nursing care in the patient's home according to the physician's orders and established plan of care

• Perform nursing assessments, monitor vital signs, and observe changes in the patient's condition

• Administer medications and treatments as ordered

• Maintain accurate and timely nursing documentation

• Communicate changes in the patient's condition to the RN supervisor and physician as appropriate

• Educate and support family members regarding the patient's care, medications, and safety

• Collaborate with the clinical team to ensure quality, patient-centered care

• Promote patient comfort, safety, and overall well-being

A Few Must-Haves for Our Licensed Practical Nurses:

• Graduate of an accredited practical nursing program

• Current, active LPN license in the state of Ohio

• Minimum of one year of nursing experience required

• Current CPR certification

• Strong clinical assessment and communication skills

• Ability to work independently in a home setting

• Dependable, compassionate, and committed to quality patient care

• Previous home health or private duty nursing experience preferred, but not required
