Care Options for Kids is seeking a Licensed Practical Nurse (LPN) to deliver focused, one-on-one pediatric home health care in Pennsylvania. You will work closely with a registered nurse and clinical leadership to provide high-quality nursing care with clear direction and support.

The role emphasizes meaningful patient relationships, stability, and a team that prioritizes nurse safety and success, with home visits under professional supervision.

Qualifications

• Valid Pennsylvania LPN License or Multistate License.
• TB Skin Test (PPD) or TB Blood Test (QF).
• Valid BLS CPR card (in-person).
• Valid driver's license.
• G-tube, trach, vent experience or willingness to train.

Responsibilities

• Provide one-on-one pediatric home health care under RN oversight.
• Follow care plans with clear clinical direction.
• Deliver care in home environments with CDC-aligned safety.
• Collaborate with clinical leadership for patient outcomes.
• Ensure accurate documentation and communication with the care team.
