Care Options for Kids is seeking a Licensed Practical Nurse (LPN) for pediatric home health in Pennsylvania. You will deliver focused, one-on-one care in the patient’s home, working under RN oversight with clinical leadership.

Qualifications

• LPN license required in Pennsylvania.
• TB test (PPD or TB blood test) required.
• BLS CPR certification required (in-person).
• Valid driver’s license required.
• G-tube/trach/vent experience or willingness to train.

Responsibilities

• Provide one-on-one pediatric home health nursing under RN oversight.
• Collaborate with clinical leadership to develop and follow plans of care.
• Deliver care in accordance with CDC safety guidelines and organizational policies.
• Establish trusting relationships with patients and families and document care.
• Support nurse safety and continuity of care in home settings.
