Job summary

Stone Rehabilitation & Senior Living is hiring a Licensed Practical Nurse to join our overnight full-time team. The position offers a $5,000 sign-on bonus and pay range of $42.00 to $48.00 per hour based on experience.

This role focuses on direct resident care, medication administration, ADLs support, and collaboration with the care team to maintain quality outcomes in a supportive environment with opportunities for per diem shifts.

Qualifications

• Active MA LPN licensure.
• Strong nursing skills in meds, vitals, and documentation in EHR.
• Knowledge of long-term care regulations and infection control.
• Current CPR certifications (BLS and ACLS).
• Ability to communicate fluently in English.

Responsibilities

• Provide direct resident care including administering medications, monitoring vital signs, and treatments.
• Assist residents with ADLs while promoting dignity and independence.
• Monitor and document residents' clinical status and progress.
• Collaborate with care teams to update individualized care plans.
• Ensure infection control and safety standards to maintain a safe environment.

Delivering Solutions that Matter:

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success:

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long‑term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English - both verbally and written
