Haven Homes, a Cassia community, is seeking Licensed Practical Nurses (LPNs) to join our caring team in Maple Plain, Minnesota. This part-time role offers day and evening shifts with competitive pay based on experience.

Responsibilities include administering medications, documenting care, and monitoring residents' conditions while collaborating with the Clinical Manager and physicians. Our workplace values teamwork and compassion.

Qualifications

• Must have MN LPN license and be in good standing.
• Prior LTC experience preferred.
• Strong communication and organizational skills.
• Self-starter with a desire to be a part of a team.

• Administers medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response.
• Manages medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary.
• Performs additional duties as required.

• Days: 6:00 AM - 2:30 PM
• Evenings: 2:00 PM - 10:30 PM

Wage Range

$28.00 - $34.66 / hour, depending on experience

Licensed Practical Nurse Responsibilities
