Polaris Healthcare and Rehabilitation Center in Milford, Delaware is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team. The LPN will provide direct patient care under RN supervision, administer medications, monitor patient conditions, and assist with IV therapy and wound care in a warm, patient-centered environment.

The ideal candidate holds an active Delaware LPN license, has LTC experience where possible, and demonstrates professionalism and reliability.
