Polaris Healthcare and Rehabilitation Center in Milford, Delaware is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team. As an LPN, you will provide direct patient care under RN supervision and participate in care planning to deliver quality resident care.

• Active Delaware LPN License required.

Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare professionals.
• Administer medications, treatments, and procedures as prescribed by healthcare providers.
• Monitor and record vital signs, medical conditions, and treatment outcomes.
• Assist with invasive and non-invasive procedures, including IV therapy and wound care.
• Collaborate with other healthcare professionals to develop and implement individualized care plans.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions.

Requirements:
