Job summary

Cedar View Rehabilitation and Healthcare Center, a 106-bed Sub-Acute and Long-Term Care facility in Massachusetts, seeks an LPN to join its compassionate team. You will deliver safe, person-centered care and work with an interdisciplinary group.

Qualifications

• Current, unencumbered LPN license in MA.
• Valid BLS Certification (CPR).
• Ability to care for and manage multiple patients in a fast-paced environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per plan of care and physician's treatment.
• Follow care plans developed by Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments per the POC and facility policies.
• Monitor residents, report changes promptly to supervising nurse or physician.
• Maintain accurate documentation per orders and standards.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of MA.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
