Job summary

Briarwood Rehabilitation and Healthcare Center is seeking an LPN to join our team. Full-time and PRN opportunities available, with Daily Pay and competitive shift differentials.

The LPN will provide patient care per the plan of care, follow care plans, administer medications, monitor residents, document accurately, and support a safe, respectful environment for residents and families. Requirements include MA LPN license, valid BLS, ability to manage multiple patients, strong clinical judgment,

Qualifications

• Current, unencumbered active LPN license in MA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted nursing standards and plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager and team.
• Perform medication administration and prescribed treatments per plan of care and facility policies.
• Monitor residents' conditions, report changes promptly, and respond to clinical needs.
• Maintain accurate documentation in compliance with physician orders and regulatory standards.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.


• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.


• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.


• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.


• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.


• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of MA.


• Valid BLS Certification (CPR) required.


• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
