Complete Care at Maple Grove in Madison, WI is seeking a dedicated Licensed Practical Nurse (LPN) for full-time 2nd shift. You will document resident care, transcribe orders, administer medications, monitor residents, and assist with wound care and procedures within your licensed scope.

Qualifications

• Active and unrestricted LPN license in Wisconsin.
• Ability to collaborate within a multi-disciplinary clinical team.
• Experience in long-term care or skilled nursing preferred; motivated new licensees encouraged.

Responsibilities

• Maintain accurate electronic documentation of resident care.
• Transcribe physician orders to MARs and notify pharmacy of changes.
• Prepare and administer medications according to facility protocols.
• Monitor residents, take vitals, escalate concerns to the RN/Unit Manager.
• Perform wound care and other clinical procedures within licensed scope.
• Provide guidance to CNAs to ensure high-quality direct care.
