Job summary

Ohioans Home Healthcare is seeking an LPN for full-time, dayshift in-home patient care across Erie, Huron, and Lorain Counties. Under RN supervision, you will assess, plan and implement care, educate patients and families, and manage medications, wounds and devices in patients' homes.

Qualifications

• Current LPN state licensure
• Three years of nursing experience
• CPR certification
• Ability to travel for in-home patient visits

• Excellent interpersonal and customer service skills
• Home healthcare experience preferred

Responsibilities

• Assess, plan and implement patient care in home health setting under RN supervision.
• Educate patients and family on disease processes, procedures and medications.
• Address patient needs including wound care, trachs, G/J tubes, Foley catheters and medication administration.
• Provide continuity of care and build long-term patient relationships.
• Document care using iPad/electronic charting and portable printers.
