Job summary

Leavitt Family Jewish Home is seeking a Licensed Practical Nurse (LPN) for the 3:00PM-11:00PM shift. This full-time role involves administering medications, monitoring residents, and supporting ADLs in a long-term care setting.

Qualifications

• Active, unencumbered MA LPN licensure.
• Knowledge of long-term care regulations and infection control standards.
• Current CPR certifications, including BLS.
• Ability to communicate fluently in English – both verbally and written.

Responsibilities

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments per physician orders and care plans.
• Assist residents with ADLs such as bathing, dressing, grooming, feeding, mobility, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including condition changes, wound care, intake/output, and responses to treatments; report significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies; maintain a clean, safe environment.

$35.00 - $39.00/hour, based on experience
Full-Time | 3:00PM - 11:00PM | Rotating weekends and holidays

Delivering Solutions that Matter:

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success:
