Trinity Health At Home in Livonia, MI, is seeking a Revisit Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients’ homes. Working under supervision, you will follow physician-approved care plans and support clients toward safe, independent living.
You will administer medications, plan visits, document care, educate families, and participate in interdisciplinary teams. Required: state LPN licensure and at least 1 year of acute care experience; homecare preferred.
