Job summary

STAT Home Health in Lake Charles, LA is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing and support to patients in their homes under the supervision of an RN.

This full-time role emphasizes compassionate in-home care, coordination with physicians and the care team, and thorough documentation. Louisiana LPN license and 2+ years of experience are preferred, with home health background advantageous.

Qualifications

• Louisiana LPN license required.
• Minimum of 2 years as an LPN.
• Home health experience preferred.
• Must have dependable transportation, driver's license, and car insurance.

Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care under RN supervision.
• Coordinate with attending physicians, home health team, and other agencies to ensure optimal patient care.
• Document all provided services.
• Demonstrate knowledge of Medicare/Medicaid home health regulations.
• Report changes in patient condition to the RN.
• Manage time effectively to meet patient needs.
