STAT Home Health in Lafayette, LA is hiring full-time Licensed Practical Nurses (LPN). You’ll provide nursing care and coordinate with the RN and physicians to ensure high-quality home health services.

Job Duties

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
