Job summary

STAT Home Health in Monroe, LA is seeking a Full-Time Licensed Practical Nurse (LPN) to join our patient-focused team. You will deliver compassionate nursing care in patients' homes, following the Interdisciplinary Plan of Care under the supervision of the RN.

Applicants must hold a Louisiana LPN license, CPR certification, and have at least one year of clinical LPN experience. You will coordinate with physicians and home health staff, document all services, and help families understand

Qualifications

• Louisiana LPN license current and in good standing.
• At least 1 year of clinical LPN experience.
• Current CPR certification.

Responsibilities

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
