Job summary

Bristol Hospice - High Desert in Klamath Falls, OR is seeking a compassionate LPN to provide direct patient care in a hospice setting. This full-time, day shift role offers a competitive hourly wage and a schedule of Monday–Friday, 8am–5pm, with coverage area Klamath County.

Qualifications

• Must currently be licensed as an LPN/LVN in the State
• Three years nursing experience; hospice or medical/surgical preferred
• Understand hospice philosophy and care needs of the terminally ill
• Flexible in work hours
• Maintain knowledge of information systems and applicable software

Responsibilities

• Provide direct patient care per the plan of care and state practice act
• Document patient services accurately and timely to reflect the plan of care
• Educate patient and family/caregiver about diagnosis and self-plan of care
• Coordinate hospice services with the interdisciplinary team
• Provide pain/symptom management and evaluate response to treatments
• Participate in ongoing professional growth and quality activities
• Perform other duties as assigned by the registered nurse

Qualifications: An Average Day:(Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
