Job summary

Alternate Solutions Health Network is seeking a Licensed Practical Nurse to provide skilled home-based nursing care in Dayton, Kettering, West Carrollton, and surrounding areas. Work Monday–Friday with on-call requirements, involving autonomous patient visits and collaboration with an interdisciplinary team.

The role requires an active LPN license in Ohio, at least one year of acute care experience, and a valid driver's license.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum one-year experience as an LPN in an acute care setting; home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe patient conditions.
• Minimize Patient Risk: Administer medications and educate patients to reduce re-hospitalizations.
• Family Educator/Advocate: Educate patient and family using teach-back methods.
• Policies: Complete documentation following agency protocol and Medicare guidelines.
• Rules and Regulations: Follow agency policies and communicate schedule changes.
• Operations: Attend in-service training and mandatory meetings.

Agency: Kettering Home Care

Schedule: Monday - Friday, 8:15AM - 5:00PM with on-call requirements

Territory: Clinicians will work in Dayton, West Carrolton, Kettering, and the surrounding areas

HOW YOU'LL WORK:

You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:

• Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient's condition.
• Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
• Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
• Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.
