Job summary

Alternate Solutions Health Network’s Kettering Home Care is seeking a Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in patients' homes, Monday through Friday, 8:30 AM to 5:00 PM. You will follow physician orders, monitor conditions, administer meds, educate families, and collaborate with a supportive interdisciplinary team.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Minimum of 1 year of LPN experience in an acute care setting.
• Home health experience preferred.
• Valid driver’s license and active auto insurance.
• Strong communication, time management, and documentation skills.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Follow physician orders and care plans created by the Case Manager.
• Monitor patient conditions and report changes.
• Administer medications and treatments as prescribed.
• Educate patients and families using teach-back methods.
• Complete clinical documentation in accordance with agency and Medicare guidelines.
• Communicate schedule or availability changes with the appropriate team members.
