Summa Home Health is seeking an Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in patients' homes in the Kettering area. You will follow physician orders, monitor patient status, administer medications, and educate families to support safe recovery.

This role emphasizes compassionate, patient-focused care, with a Monday–Friday, 8:30 AM–5:00 PM schedule and a supportive interdisciplinary team to improve patient outcomes.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Minimum of 1 year of LPN experience in an acute care setting.
• Home health experience preferred.
• Valid driver’s license and active auto insurance.
• Strong communication, time management, and documentation skills.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide skilled nursing care in patients’ homes.
• Follow physician orders and care plans created by the Case Manager.
• Monitor patient conditions and report changes.
• Administer medications and treatments as prescribed.
• Educate patients and families using teach‑back methods.
• Help reduce hospital readmissions through patient education and preventative care.
• Complete clinical documentation in accordance with agency and Medicare guidelines.
