Job summary

Alternate Solutions Health Network is seeking a Registered Nurse to provide one‑on‑one home health care in our Warren, Clermont, Brown, Highland, and Clinton County territories. The schedule is Monday, Wednesday, Friday, 2:00 PM–9:00 PM, with no on‑call requirements.

You will lead patient care at the home, work autonomously, and collaborate with an interdisciplinary team committed to quality outcomes. You must hold an active RN license in the state, have at least one year of acute care

Qualifications

• Registered Nurse license active in the state of employment.
• Minimum one year of RN experience in an acute care setting; home care experience preferred.
• Valid driver’s license and auto insurance in your name.
• Capable of all physical demands as required by home health care.

Responsibilities

• Plan of Care: Observe and monitor patient conditions and perform OASIS assessments to develop a personalized care plan.
• Minimize patient risk by administering medications as prescribed and prioritizing high‑risk patients.
• Coordinate multi-disciplinary care to ensure continuity of care during admissions, transfers and discharges.
• Educate patients and families on disease processes using teach‑back methods to ensure understanding.

HOW YOU'LL WORK:

You will provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You will be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:

• Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you do not already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change.
• Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease rehospitalization by prioritizing visits for high-risk patients.
• Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding.
• Policies : Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided.
• Rules and Regulations : Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

REQUIREMENTS:

• Registered Nurse with current license in the state of employment
• Minimum one-year experience as an RN in an acute care setting Home Care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
