Job summary

Reliant Care Management Compan in Kansas City, MO is seeking FT and PRN LPNs for day and night shifts. You will conduct assessments, follow protocols, document changes, manage admissions and discharges, and lead CNAs and CMTs to ensure safe, compliant care. A $10,000 sign-on bonus is offered.

This role requires a valid LPN license and a compassionate, patient-centered approach. You will provide wound care, respond to emergencies, and adhere to facility policies while supporting your team.

Qualifications

• Must hold an LPN license and be in good standing.
• Demonstrated compassion and patient-centered care.
• Ability to lead CNAs and CMTs while following facility protocols.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
