Birmingham Rehabilitation and Wellness Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Forestdale, AL. The LPN will provide high-quality patient care within a dynamic rehabilitation setting and work closely with a multidisciplinary team to support patient recovery.

Responsibilities include administering medications, wound care, vital signs, and contributing to discharge planning while maintaining accurate records.

Qualifications

• Current LPN licensure in Alabama.
• CPR Certification is required.

Responsibilities

Shifts Available: Monday-Friday 2:30pm-11:00pm/Weekends 6:30pm-7:00am with baylor plan

Job Summary:

Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an integral part of our interdisciplinary team, the LPN will provide high-quality patient care and support in a dynamic and fast-paced rehabilitation setting.

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date patient records and documentation
• Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care
• Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation
• Maintain a safe and clean environment for patients, staff, and visitors

Requirements:

• Current LPN licensure in the State of Alabama
• CPR Certification
