Interim HealthCare of the Twin Cities is seeking a Licensed Practical Nurse (LPN) to provide in-home care, care coordination, and patient education. You will assess, monitor, and support patients in their own homes, collaborating with RNs and physicians to implement the plan of care.
This full-time role requires MN LPN license, CPR, valid driver's license, and ability to travel within the southeast metro. Expect regular day shifts, rotating weekends/holidays, competitive pay, and benefits
Home Care Licensed Practical Nurse (LPN): Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holiday's per year. The weekend and holiday rotation is dependent upon staffing levels.
This position will work in the field helping patients in their home setting. Our office is inRoseville, MN and the general territory for this position would be the southeast metro.
