Job summary

Interim HealthCare of the Twin Cities is looking for a Home Care Licensed Practical Nurse (LPN) in Inver Grove Heights, MN. This position offers independence and flexibility to provide one-on-one patient care in their home environment while coordinating healthcare needs.

Qualifications

• Current license as a Licensed Practical Nurse (LPN) in the State of MN; license is active and in good standing.
• Preferred: 2 years experience as an LPN.
• Must be able to travel within the geographic area serviced by the office; valid driver’s license, reliable transportation, and proof of auto insurance required.
• CPR Certification.

Responsibilities

• Participate in ongoing interdisciplinary assessment of the patient.
• Provide services that are ordered by the physician as indicated in the plan of care.
• Recognize and report changes in the patient’s condition to the RN.
• Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Counsel and educate the patient and family regarding healthcare related needs.
• Complete timely and accurate clinical notes including patient progress.

Home Care Licensed Practical Nurse (LPN)

Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Hours

Full‑time. Monday through Friday days. There will be a weekend and holiday rotation; work approximately one weekend every 4‑6 weeks and two holidays per year, depending on staffing levels.

Territory

The position works in the field helping patients in their home setting. The office is in Roseville, MN and the general territory is the southeast metro area.
