Role Summary

Plans and manages patient care according to each patient's needs. Conducts patient interviews, records medical history and physical condition, obtains vital signs, and provides routine care.

Responsibilities

  1. Provide 1:1 care for clients, following their care plans while maintaining dignity and independence.
  2. Monitor vital signs and report abnormalities.
  3. Administer medications, feedings, oxygen, and ostomy care as per care plans.
  4. Coordinate with other care professionals and complete routine assessments.
  5. Maintain infection control policies and report communicable diseases.
  6. Document nursing actions and communicate with families and healthcare team.
  7. Order supplies and medications to ensure availability.
  8. Ensure patient needs are met according to physician instructions.
  9. Monitor incidents and complete incident reports timely.
  10. Maintain medical equipment and report faults.
  11. Perform other duties as assigned.

Qualifications

  1. Completed post-secondary coursework for licensure as LPN or LVN.
  2. Current license in good standing.
  3. CPR Certification.
  4. Excellent interpersonal skills, patience, empathy.
  5. Knowledge of patient care policies and regulations.
  6. Understanding of symptoms and signs of medical conditions.
  7. Moderate travel between facilities may be required.

Open interviews are held every Thursday from 10am to 2pm at 8041 KNUE RD, INDIANAPOLIS, IN 46250. Salary range: USD $30.00 - $30.60 per hour.
