Linden Place Center for Nursing is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team in North Carolina. You will provide hands-on resident care under the supervision of the Registered Nurse, administer medications, monitor conditions, document accurately, and support coordinated, resident-centered care plans.

Qualifications

Active, unrestricted LPN license in NC.
Current CPR and First Aid certification preferred.

Responsibilities

Provide direct nursing care under the supervision of the RN and clinical leadership.
Administer medications and treatments safely and accurately in accordance with facility policies.
Monitor resident conditions and promptly report changes or concerns to the RN.
Perform routine clinical duties including vital signs, wound care, catheter care, and specimen collection.
Assist with admissions, transfers, and discharges as needed.
Reinforce resident and family education as directed by the RN.
Maintain accurate, timely, and complete clinical documentation.
Support individualized care plans in collaboration with the interdisciplinary team.
Promote resident safety, comfort, dignity, and overall well-being.
