Job summary

Trinity Health Of New England At Home – Connecticut is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes in the Hartford area. You will follow the plan of care prescribed by the physician and coordinated by a supervising RN.

This full-time position runs Monday through Friday, 8:00 am to 4:30 pm with weekend/holiday rotation as required. Home healthcare experience is preferred and driving to patient locations is expected.

Qualifications

• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the state of practice.
• Minimum of one (1) year experience in acute care.
• Home healthcare experience preferred.
• Must possess interpersonal skills to interact with clients, families, and caregivers.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation.
• Must demonstrate the Organizational Core Competencies.

About The Position

Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.

Monday-Friday, 8a-4:30p, weekend/holiday rotation as required

Minimum Qualifications

• Graduate from an accredited or state approved school of practical nursing
• Current licensure in the state of practice
• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
• Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.
