Job summary

Walter Reed Post Acute is hiring licensed practical nurses (LPNs) to join our 181-bed sub-acute and long-term care facility in Virginia. Full-time and part-time opportunities are available, with evening or night shifts and a shift differential to recognize your dedication.

You will provide patient care per plan of care, administer medications, document accurately, and collaborate with an interdisciplinary team to support residents’ physical and emotional needs.

Qualifications

• Current, unencumbered active LPN license in Virginia.
• Valid BLS Certification (CPR) required.
• Ability to manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills with attention to detail.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow individualized care plans developed by the Nurse Manager and team.
• Administer medications and prescribed treatments per policy.
• Monitor residents, report changes promptly, respond to clinical needs.
• Maintain accurate, timely documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):
