Job summary

Award Care Health Group is seeking a qualified Licensed Practical Nurse (LPN) to join our family-oriented facility. We welcome new grads and offer full-time and part-time shifts (7a–7p and 7p–7a).

A bi-weekly bonus of $225 is available through the Perfect Punch Program, reflecting our commitment to recognizing dedicated care. As an LPN, you will provide direct patient care in a skilled nursing/rehabilitation setting, support treatment plans, supervise CNAs, and monitor falls and pressure sore

Qualifications

• Active LPN license in good standing.
• IV certification preferred.
• Experience in skilled nursing/rehabilitation facilities preferred.
• New grads welcome.

Responsibilities

• Provide direct patient care in a skilled nursing/rehabilitation center.
• Assist in implementing individualized treatment plans.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore Programs.
• Conduct and document evaluations of residents' medical status.
• Perform other duties as assigned.

Major Responsibilities

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment.
• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• IV Certified preferrable
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
