Job summary

Interim HealthCare of Augusta is seeking a Home Health Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care in patients' homes in Gastonia and Shelby. Set your own schedule with flexible, autonomy-rich assignments that fit your life.

As part of a nurse team, you will take vitals, assess conditions, document progress, and educate patients and families on plans of care and home safety. We support growth with online training, CEUs, and a connected, supportive workplace.

Qualifications

• Graduate of an accredited practical nursing program
• Active LPN license in NC
• Clinical proficiency in nursing and strong communication

Responsibilities

• Provide home-based care to patients under the supervision of an RN
• Collaborate with a team of physicians, RNs, CNAs and aides
• Take patient vitals, perform assessments, document progress and report changes to RN
• Assist RN with patient's plan of care and teach self-care techniques
• Educate family on the plan of care, medication and home safety
• Coordinate patient care and discharge planning with interdisciplinary team

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Licensed Practical Nurses:

• Graduate of an accredited practical nursing program and active LPN license in NC
• Clinical proficiency in nursing, strong communication skills and compassion
