Job summary

Easterseals Florida is seeking a compassionate LPN to join our Adult Day Healthcare program in Gainesville. You will provide health monitoring and nursing services for participants in the program, contributing to a person-centered setting that enhances quality of life.

Qualifications

• Graduate of an accredited nursing program.
• Current Florida LPN license.
• At least 1 year experience with individuals with disabilities or aging population.
• Ability to assist transfers and activities of daily living.
• CPR and first aid certification a plus (can be obtained after hire).

Responsibilities

• Coordinate admissions and discharges, and maintain participant charts and medical records.
• Schedule in-house medical visits, appointments, and transportation as needed.
• Track and coordinate quarterly care plan meetings with participants and family.
• Take, document, and maintain records, reporting trends to the Center Director.
• Provide clinical support and in-service training to program staff.
• Educate participants and families on health, meds, safety, emergencies, and resources.

Easterseals Florida is seeking a compassionate, professional and dynamic Nurse (LPN) to join our team in our Adult Day Healthcare program in Gainesville. With an average daily attendance of 25 clients, you'll be part of a caring, fun environment. In this position, you will perform all health monitoring and nursing services for participants in the program. The nurse provides excellent care and customer service in a person-centered setting designed to enhance the quality of life for our participants.

A snapshot of what you'll do:

• Assist in coordinating admissions and discharges, building new participant charts/medical records, ensuring admissions paperwork is completed.
• Assist in scheduling in-house medical specialty visits, medical/health appointments, and transportation as needed
• Track, review, and coordinate quarterly care plan meetings with participants and family/caregiver team
• Take, document, and maintain records for each participant, reporting trends outside of parameters, or needing attention from the Center Director
• Provide clinical support, insight, knowledge, and applicable in-service training to program staff.
• Provide education to participants and family/caregiver team on health and wellness, medical diagnosis', medication, safe living, emergency situations, and resources available.

Qualifications:

• Graduate of an accredited nursing program
• Current Florida Licensed Practical Nurse License
• At least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patients
• Ability to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc)
• CPR and first aid certification a plus, but can be obtained/renewed once hired
