Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a dedicated LPN to join our compassionate team. You will provide direct patient care, administer medications, monitor vital signs, and assist with basic wound care to support residents’ well-being.

You will document patient information, communicate with families and healthcare providers, and participate in interdisciplinary rounds to ensure holistic care. Prior long-term care experience is preferred but not required.
