Job summary

Illuminus in Fond du Lac, WI is seeking a Licensed Practical Nurse (LPN) to deliver resident care under RN supervision, including medication administration, monitoring and documentation, to provide person-centered care.

We offer full-time, part-time, and PRN opportunities with 8-hour shifts and weekend rotation; pay ranges from $26.55 to $33.68 per hour, based on experience. Illuminus is a faith-based, not-for-profit senior living organization serving older adults across the Midwest.

We offer full-time, part-time, and PRN opportunities with a variety of scheduling options. Shift availability may vary based on the needs of the community.

This position may include 8-hour shifts, including weekend rotation as scheduled.

Essential Job Functions

Our Licensed Practical Nurse (LPN) provides prescribed medical treatment under the general supervision of a registered nurse.

• Maintains professional communication with residents, families, and staff while promoting the organization’s mission, values, and positive community image.
• Develops and implements care plans; administers medications and treatments; monitors resident health; assists with physician visits; and verifies physician orders.
• Leads and supervises CNAs and resident assistants, promotes quality resident-centered care, and follows safety, infection control, and PPE requirements.
• Observes, documents, and reports resident status and changes in condition; completes required documentation and communicates concerns and care information to the appropriate nurse, family, or physician.

Requirements

• Current Licensed Practical Nurse credentials and license in the State of Wisconsin.
• Active CPR

• Ability to relate well to all individuals.
• Ability to maintain and protect the confidentiality of information.
• Ability to exercise independent judgment and make sound decisions.
