Job summary

Allens Cove Health Care Center in Pennsylvania is seeking a dedicated LPN to join our caregiving team. The ideal candidate will combine empathy with clinical competence to assess and document resident status, perform admission and discharge tasks, and coordinate care with physicians and guardians.

You will provide wound care, follow protocols, and respond promptly to emergencies while leading CNAs and CMTs in delivering high-quality nursing services in a collaborative environment.

Qualifications

• Active LPN license is required.
• Candidates should demonstrate a caring, empathetic approach to residents.
• Willingness to care for residents and collaborate with the care team.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
