Beaumont Home Health is seeking an LPN to provide high-quality, home-based nursing care to patients under the direction of the attending physician. You will work autonomously in patients’ homes, following order sets and goals created by the Case Manager, while engaging with patients’ families to support recovery.

This role requires a minimum of one year of LPN experience in acute care, a valid Michigan driver's license, and the ability to handle the physical demands of home care.

Formación

• LPN license in the state of employment.

• Valid driver’s license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsabilidades

• Provide skilled nursing care in patient’s home as ordered by the physician.
• Administer medications and monitor patient conditions per orders.
• Educate patient and family using teach-back methods.
• Complete clinical documentation following agency and Medicare guidelines.
• Respect policies, communicate schedule changes to schedulers.
• Attend in-service trainings and mandatory meetings.

You’ll provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager. HOW YOU’LL WORK:

You’ll provide skilled nursing care in a patient’s home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You’ll be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:

• Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition.
• Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
• Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
• Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

REQUIREMENTS:

• Licensed Practical Nurse (LPN) with current license in the state of employment
• Minimum one-year experience as a LPN in an acute care setting. Home care experience preferred.
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands
