Job summary

Sage Specialty Hospital in Denham Springs is seeking a Full-time LPN to provide nursing and supportive care. This role involves working under the direction of an RN and includes responsibilities such as coordinating patient care and documenting services.

Candidates should have at least 1 year of experience in an LTAC or Surgical unit and be able to work a flexible schedule, including 12-hour shifts. The position offers a $2,500 sign-on bonus and an Employee Stock Ownership Plan (ESOP) among other benefits.

Qualifications

• Must be able to work flexible schedule including 12 hour shifts.
• Have dependable transportation, driver’s license and car insurance.

Responsibilities

• Provides nursing and supportive care according to the Interdisciplinary Plan of Care.
• Communicates with physicians and team members to coordinate optimal care.
• Documents all services provided and submits necessary forms.
• Demonstrates knowledge of Medicare/Medicaid regulations.
• Reports changes in patient condition to RN.
