Job summary

Sage Specialty Hospital (LTAC) in Denham Springs, LA is hiring full-time LPNs for day and night shifts. You will provide nursing and supportive care under the Interdisciplinary Plan of Care, coordinating with RNs and physicians to ensure optimal patient outcomes.

Qualifications

• Minimum 1 year experience in LTAC or surgical unit.
• Willing to work 12-hour shifts.
• Must have dependable transportation, driver’s license and car insurance.

Responsibilities

• Provide nursing and supportive care per Interdisciplinary Plan of Care under the direction of the RN.
• Coordinate care with attending physicians, hospice team, and other agencies.
• Document all services provided and submit on appropriate forms.
• Demonstrate knowledge of Medicare/Medicaid hospice benefits.
