Job summary

Nuclear Care Partners is seeking a Licensed Practical Nurse (LPN) to deliver compassionate, in-home nursing care in Cortez, CO. You will work with a nurse-led team to develop and execute patient-centered care plans right in patients' homes.

Shifts are Part Time and PRN with benefits including PTO, insurance options for full-time staff, a 401(k), EAP, and opportunities for mentorship and growth in home care.

Qualifications

• Current LPN license in CO is required.
• CPR certification is required.
• Valid driver's license with reliable, insured vehicle.
• Home health experience preferred but not required.
• Must pass pre-employment screenings.

Responsibilities

• Provide skilled, in-home nursing care to assigned patients.
• Develop and follow individualized care plans.
• Document patient care, observations, and updates.
• Collaborate with interdisciplinary care teams.

Overview

At Nuclear Care Partners, we provide in-home nursing care to individuals impacted by their work in the Department of Energy or Nuclear Energy programs. As a Licensed Practice Nurse (LPN), you will make a meaningful impact by delivering skilled and compassionate, one-on-one, patient centered care directly in patients' homes. In this role you will be supported by a mission-driven, nurse-led team that values your contribution and prioritizes patient dignity and outcomes.

Compensation

$21.00 - $29.96 based on experience.

$3/hr shift differential for nights and weekends.
