Job summary

Nuclear Care Partners is seeking a Licensed Practical Nurse (LPN) to provide skilled, in-home nursing care for former DOE/Nuclear Energy program participants in Simi Valley, CA. You will work one-on-one with patients, delivering compassionate care and developing individualized care plans.

You will be supported by a nurse-led team focused on dignity and outcomes, with scheduling flexibility and opportunities for growth in in-home care.

Qualifications

• Current LPN license in the state of CA.
• Current CPR certification.
• Valid driver's license and reliable, insured vehicle.
• Home health experience preferred but not required.
• Must pass pre-employment screenings.

Responsibilities

• Provide skilled, in-home nursing care to assigned patients.
• Develop and follow individualized care plans.
• Accurately document patient care, observations, and updates.
• Collaborate with interdisciplinary care teams.
• Maintain compliance with DOL, CMS, and state regulatory requirements.

Overview

As a Licensed Practice Nurse (LPN), you will make a meaningful impact by delivering skilled and compassionate, one-on-one, patient centered care directly in patients' homes. In this role you will be supported by a mission-driven, nurse-led team that values your contribution and prioritizes patient dignity and outcomes.

Compensation

$24.00 - $35.00

Compensation is based on experience, shift type, and market factors. The listed range reflects entry-level to highly experienced applicants.
