Job summary

Cnscares is seeking a Licensed Practical Nurse to provide compassionate in-home care for patients. The role requires regular visits, adherence to the patient’s Plan of Care, and a focus on safety and independence in the home setting.

Ideal candidates have at least 2 years of in-home health experience, an active LPN license, CPR/BLS, TB testing, and a valid driver’s license. Every-other-weekend shifts and travel within 60 miles are expected.

Qualifications

• 2 years in-home health care experience.
• Must have every other weekend availability (Saturday and Sunday).
• Must be willing to travel up to 60 miles.

Responsibilities

• Provides direct patient care as described in state regulations for LPN.
• Follows the Plan of Care to provide safe, competent care/services to the patient.
• Maintains a healthy & safe environment.
• Evaluates the patient’s needs on a regular basis.
• Encourage the patient's independence but not exceed their capabilities outlined in the Patient's Care Plan.
• Other duties as assigned.

Pay Rate:$32.00-$36.00 per hour

Shift: 12-hour Shifts; Full Time or Part Time

**Every other weekend required**

Essential Functions:
