Role Responsibilities:

1. Demonstrates knowledge of medications and their correct administration based on patient age and clinical condition.
2. Performs patient care considering age-specific needs, including geriatric and general inpatient rehabilitation patients.
3. Assists with patient assessments and reassessments under the supervision of an R.N., and initiates appropriate rehabilitation nursing interventions.
4. Performs timely and accurate quality improvement assessments under supervision.
5. Implements the plan of care based on patient needs and responses, evaluating and updating the interdisciplinary care plan as needed.
6. Interacts professionally with patients and families, involving them in the care planning process.
7. Assesses and manages pain effectively, educating patients and families about pain management techniques.
8. Performs other duties as assigned.

Minimum Qualifications:

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Current state licensure as a Licensed Practical/Vocational Nurse.
• IV Certification required or obtained within 6 months (highly preferred).
• Minimum six months of Medical/Surgical experience in an acute care setting (preferred).
• Excellent communication and interpersonal skills.
