Licensed Practical Nurse (LPN) -$5000 Sign On Bonus
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our nursing team at Woburn Rehabilitation and Nursing Center, the original Salter Healthcare location, with 140 beds. As an LPN, you will provide direct resident care, supervise nursing support staff, and help ensure high-quality, resident-centered care in accordance with facility policies, clinical standards, and regulatory requirements.
Responsibilities
- Direct the daily activities of Certified Nursing Assistants (CNAs) and other nursing personnel on assigned units.
- Ensure compliance with facility policies, procedures, and applicable federal and state regulations.
- Participate in resident admissions, discharges, and transfers.
- Observe, document, and report changes in residents' conditions to physicians, families, and nursing leadership.
- Accurately transcribe and carry out physician orders.
- Prepare and administer medications and monitor residents for adverse effects.
- Provide wound care, colostomy changes, tube feedings, suctioning, IV administration, and other assigned skilled nursing interventions.
- Conduct regular rounds to ensure resident needs are being met and staff are performing assigned duties appropriately.
- Collaborate with the interdisciplinary care team to provide comprehensive and holistic resident care.
- Initiate, review, and update resident care plans as needed.
- Provide end-of-life care as required.
- Maintain adequate stock of medications, supplies, and equipment and notify appropriate personnel of needs.
- Promote teamwork, mutual respect, and effective communication among staff.
- Participate in surveys, inspections, and quality assurance activities.
- Protect resident dignity, rights, privacy, and confidentiality at all times.
Qualifications
- Current, unencumbered Massachusetts LPN license.
- Strong communication and critical-thinking skills.
- Ability to handle challenging situations with tact and professionalism.
- Ability to work effectively with residents, families, and interdisciplinary team members.
- Prior long-term care experience preferred but not required.
