Port Rehabilitation and Healthcare Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Newburyport, Massachusetts.

What You’ll Do

• Provide compassionate, high-quality nursing care to residents in a skilled nursing and rehabilitation setting

• Administer medications and treatments according to physician orders and facility policies

• Monitor residents’ conditions and report changes promptly to the appropriate clinical team members

• Perform vital signs, assessments, wound care, and other nursing duties within the LPN scope of practice

• Assist with developing, implementing, and updating individualized care plans

• Accurately document resident care, medications, treatments, and changes in condition

• Communicate effectively with residents, families, physicians, RNs, CNAs, and other members of the interdisciplinary team

• Support a safe, clean, and respectful environment for residents

Qualifications

• Current Massachusetts LPN license

• High school diploma or equivalent

• Strong communication and interpersonal skills

• Compassionate, dependable, and committed to resident-centered care

• Ability to work effectively as part of a multidisciplinary team

• Long-term care, skilled nursing, or rehabilitation experience is a plus, but not required
