Job summary

Aspire Senior Living Carthage is hiring a licensed practical nurse (LPN) to join our compassionate team in Carthage, MO. This position offers Full-Time Day and Part-Time Evening/Night shifts with rotating weekends.

You will provide direct nursing care, administer medications, and document care plans for residents. Ideal candidates hold Missouri LPN licensure, a high school diploma or equivalent, and 1+ years of LPN experience.

Qualifications

• 1+ years LPN experience preferred.

Responsibilities

• Provide direct patient care, med administration, wound care as prescribed.
• Monitor and report changes to Healthcare Team.
• Maintain accurate resident records ( histories, plans, progress notes ).
• Collaborate with other healthcare professionals to develop individualized care plans.
• Educate residents and families on disease management, medication use, and other health topics.
• Participate in quality improvement initiatives to ensure high standards of care.

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed

• Monitor and report changes in resident condition to the Healthcare Team

• Maintain accurate and up-to-date resident records, including medical histories, treatment plans, and progress notes

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Educate residents and their families on disease management, medication use, and other health-related topics

• Participate in quality improvement initiatives to ensure the highest standards of care and service

Qualifications:

• Current licensure as a Licensed Practical Nurse (LPN) in the State of Missouri

• High school diploma or equivalent required; diploma or associate's degree in nursing preferred

• 1+ years of experience as a Licensed Practical Nurse (LPN) preferred

• Ability to lift, transfer, and reposition residents; stand for long periods; and perform other physical demands of the job
