Allwel is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Home Care team. This is a supervisory position focused on supporting and overseeing the work of Personal Care Aides and Home Health Aides who provide care to clients in their homes. The LPN will work under the direction of our Registered Nurses to ensure high-quality, person-centered care.

Part-time or Full-time

Schedule:

Day shifts, on-call and occasional weekends

Key Responsibilities:

• Supervise and support PCAs and HHAs in the field, ensuring compliance with care plans and agency standards
• Conduct routine in-home aide visits to observe and document care delivery
• Assist in training, mentoring, and evaluating aides
• Report changes in client condition to supervising RN or care team
• Maintain accurate records and documentation in accordance with agency protocols
• Ensure a safe, respectful, and supportive environment for both clients and staff

Qualifications:

• Current and valid NYS LPN license
• Minimum 1 year of experience in home care, long-term care, or a related healthcare setting
• Strong interpersonal and communication skills
• Ability to work independently in the field and make sound decisions
• Reliable transportation and valid driver’s license required
